It's described as pungent-tasting and very warm, used to send rebellious qi downward, warm the middle, dispel phlegm, and clear wind-cold from the interior, easing cold pain in the chest and abdomen and cold-type dysentery. Li Shizhen adds an unusually personal note: he loved the spice as a young man and suffered yearly eye trouble he didn't connect to it, until quitting pepper stopped the problem; a contemporary, Zhu Zhenheng, warned that its fiery, drying nature can quietly damage the qi of the spleen, stomach, and lungs over time.
